RESTORE2 clinical monitoring

We were the first home care provider in Nottinghamshire to implement RESTORE2. Our carers are trained to spot the soft signs of physical deterioration and escalate to GPs with structured, clinical-grade observations.

What this means in practice

Catching problems before they become emergencies

When someone's health starts to decline, the signs are often subtle. A slight change in breathing. Skin that looks a bit paler than usual. Eating less. Being more confused or drowsy than normal. A family member might notice these things and think ‘something is not quite right’ but not know what to do with that feeling. A GP visit might not seem justified yet. And so nothing happens until the situation becomes urgent.

RESTORE2 changes that. It gives our carers a structured framework for observing and recording physical changes. Instead of relying on a vague sense that something is off, they measure specific indicators: respiratory rate, pulse, temperature, blood pressure, skin colour, level of consciousness and appetite. These observations are scored, and the score tells them whether the situation needs escalation.

A phone call that says ‘RESTORE2 score has increased from 2 to 6, respiratory rate is elevated, appetite has dropped over three days’ gets a very different response from a GP.

Evidence GPs can act on

Most hospital admissions for older people are preceded by days or even weeks of subtle decline that nobody acted on. The person was ‘not quite themselves’ but there was no clear trigger to call a doctor. RESTORE2 provides that trigger. It turns subjective concern into objective evidence that a GP can assess and respond to.

Who this is for

RESTORE2 monitoring is included as part of our care service. You do not need to request it separately or pay extra for it. If your family member is receiving regular visits from us, RESTORE2 observations are built into their care plan.

It is particularly relevant for people with long-term health conditions: heart disease, COPD, diabetes, kidney problems, where early detection of changes can prevent hospital admissions. It is also valuable for clients who live alone and might not have anyone to notice that they are deteriorating.

If you have a family member who has been in and out of hospital and you are worried about the pattern repeating, RESTORE2 gives us a structured way to catch problems earlier. That does not mean we can prevent every admission, but it means we are actively looking for the warning signs instead of waiting for a crisis.

Structured clinical observations

Every visit includes observations of respiratory rate, pulse, temperature, blood pressure, skin colour, consciousness and appetite. These are scored against the RESTORE2 framework so nothing is left to guesswork.

Early warning signs caught sooner

Subtle changes like slightly elevated blood pressure, increased drowsiness or reduced appetite are recorded and tracked over time. Patterns that would otherwise go unnoticed trigger a structured response.

Escalation protocols that GPs trust

When a score rises above the threshold, our clinical lead reviews the data and contacts the GP with specific, measurable observations. This is evidence a clinician can act on, not a vague concern.

Data shared with clinical teams

All RESTORE2 observations are shared with district nurses, GPs and hospital teams when a client is reviewed or admitted. Clinicians see a history of observations taken in the client’s normal environment.

Every carer who works for Your Home Care receives RESTORE2 training as part of their induction. This is not a one-off session they forget about. It includes practical training on taking observations, understanding what the scores mean, and knowing when and how to escalate. We also run regular refresher training to keep skills sharp.

All observations are visible to families through the care app. You can see the readings, the scores, and any escalation notes. If your family member's blood pressure has been creeping up over a fortnight, you will see that trend in the data. This transparency means you are not relying on someone to phone you when something changes. You can see it for yourself.

RESTORE2 is a physical deterioration and escalation tool designed for care settings. It gives care staff a structured way to observe changes in a person’s health, such as breathing rate, skin colour, temperature, appetite and alertness, and decide whether to escalate to a GP or clinical team. It was developed by NHS clinicians and is recommended by Skills for Care.

Most care staff rely on instinct and experience to spot when something is wrong. RESTORE2 adds structure to that process. Instead of saying ‘she does not seem right today’, a carer can record specific observations, including respiratory rate, skin colour and level of confusion, and use a scoring system to determine whether the change warrants clinical attention. It turns a gut feeling into evidence that a GP can act on.

Yes. Our RESTORE2-trained carers can take basic observations including blood pressure, pulse, temperature and respiratory rate. These observations are recorded and used as part of the RESTORE2 assessment. They are not replacing district nurses or clinical staff. They are providing early-stage observations that can trigger timely clinical intervention.

Yes. In one case, a carer’s RESTORE2 observations identified high blood pressure in a client who had no symptoms. The structured escalation to the GP led to medication being adjusted before the situation became dangerous. Without those observations, the problem would not have been picked up until something went wrong. This is not an isolated example. Early detection is exactly what RESTORE2 is designed to achieve.

Yes, and they will be comfortable with it. The observations are non-invasive and take only a few minutes. Most clients barely notice. It becomes part of the normal visit routine. We explain what we are doing and why, and the information is shared with the family through the app.
